This gazebo was completed as a Rotary Club of La Porte service project.  It is located in the Rotary Gardens section of Soldiers Memorial Park in La Porte, Indiana

COMMUNITY ASSISTANCE APPLICATION

The Rotary Club of La Porte receives many requests each year from worthy causes throughout our community. As much as we would like to help all who ask, our financial capabilities are limited.   Therefore, we generally support programs that will have widespread benefits and match the goals of Rotary.

To complete a community assistance application, please e-mail us with the following information:

• Organization Address

• Contact Person Name and Phone Numbers

• Number of Local Members, if membership organization

• Approximate Annual Operating Budget

• Number of local citizens this request will benefit

• Names of other organizations supporting this effort

• Amount of your request

Please provide additional information that you believe will assist us in considering your request.  For example, explain the effect of this award on the intended group or community, provide background information on your organization, or let us know if someone from your organization might be interested in joining the Rotary Club of La Porte.  Thank you for your request.
